Our hybrid care model includes 30% in-person, high-touch clinical care. For patients who find travel difficult, or for procedures that require a steady hand (such as wound debridement or injections), we bring our medical bag directly to you.

What to Expect

During an in-home visit, you'll receive the same un-rushed, individualized attention you'd experience via our telehealth platform. Our clinician will perform physical examinations, review your home environment for safety/health optimization, and administer necessary treatments or tests.
